Early Adopter Trust Offers Blueprint for NHS 10 Year Plan
TPP and SWYFT share lessons from real-world delivery of neighbourhood care
South West Yorkshire Partnership NHS Foundation Trust (SWYFT), in collaboration with TPP, is sharing its experience of delivering a neighbourhood care model that aligns closely with the ambitions of the NHS 10 Year Plan – and has been in place since well before the plan’s formal release.
Since March 2020, the Trust has transitioned 23 services – including nursing, therapy, virtual wards, and specialist community care – into an integrated neighbourhood model using core functionality in SystmOne, TPP’s electronic patient record system.
Built entirely within the existing SystmOne platform, the model demonstrates how neighbourhood care can be delivered without new systems or integrations, using tools already available across the NHS. The approach focused on:
- Smarter use of referral allocation, rotas, and dashboards
- Improved internal communication and visibility across teams
- Standardised templates and workflows across services
- A gradual, service-by-service implementation strategy
Staff were re-trained to shift from caseload-based working to a referral-led model, with digital tools underpinning new collaborative ways of working. By shifting from traditional caseloads to a referral-driven model, they unlocked real-time visibility and seamless coordination across clinical teams. Using core SystmOne tools – dashboards, rotas, templates, and planners – they built a digitally standardised, scalable model of neighbourhood care without any additional systems or integrations.

The Trust has already reported benefits such as:

- Consistent digital processes across different teams
- Improved visibility of patient pathways
- More joined-up, coordinated care delivery

Paul Hughes, the Services Development Manager at South West Yorkshire Partnership NHS Foundation Trust, said: “The integration of our physical health services into one single SystmOne unit has resulted in services being able to easily see which other service interventions the patient is currently receiving. This facilitates better co-ordinated care through enhanced record sharing, resulting in improved clinical decision making and patient care being delivered.”
This model is being shared as a practical, real-world example of how NHS organisations can begin delivering the 10 Year Plan today – using tools already in place.
