Leeds Community Healthcare NHS Trust Drives Innovation with SystmConnect to Enhance Digital Patient Pathways

Leeds Community Healthcare (LCH) NHS Trust is leading the way in digital healthcare innovation with the successful implementation of SystmConnect, a comprehensive triage platform designed to streamline patient pathways and improve access to services.

Serving a diverse population of 800,000 across Leeds, West Yorkshire, LCH provides a wide range of integrated healthcare services including physical health, sexual health, mental health and child health services. With a commitment to innovation and patient-centred care, the Trust is embracing cutting-edge digital tools to improve service delivery and patient experience.

Enabling digital-first care pathways for pathways such as contraception requests and chlamydia treatment, LCH is charging the way to delivering a more modern NHS. The rollout of SystmConnect has transformed the service, increasing care accessibility and driving significant time savings. In just 10 months, SystmConnect has processed thousands of patient requests across the service, saving more than 1,000 hours of clinical and admin time, which has now been redirected to frontline patient care.

“SystmConnect aligns with our current needs and continues to develop in ways we find increasingly useful” said David Roberts, Clinical Systems Product Specialist at LCH.

“Unlike other products, we have complete control over the patient-facing forms. We’ve improved service accessibility to patients by removing barriers for patients wanting to access healthcare. If you can prevent patients having to come to the clinic, you can reduce some of the hidden patient costs of access such as issues with childcare, needing time off work, and travel costs. Digital-first services like this make healthcare more accessible for everyone.”
